Things to Know:

• It is the new patient’s responsibility to fill out your name on the referral source.

• There are NO maximum number of entries. The more you refer, the more entries, the higher your chances of winning.

• Winner will be announced on our Facebook page @glennreitdds and our instagram @reitdds. Winner will be contacted by phone, email (if provided) and mail. Winner must come to the office to pick up the gift card and be ok to take a photo only for our Facebook/ Newsletter. Winner must claim prize within 14 days.
