For many of us at some time in our lives a tooth has deteriorated to a point where a dental filling can no longer be placed.  A more elaborate tooth restoration has been performed.   This is a dental crown sometimes considered a dental cap by the public.   The crown restores your tooth to function but it is cemented onto your tooth.

Life happens and cements do fail and your crown is suddenly off.  Your tooth has lost its head.  It needs to be reattached as soon as possible.  The exposed tooth is not as strong as other teeth since the protective enamel layer has been removed during the tooth preparation for the crown. 

Sometimes a crown comes off due to a new cavity that has formed at the margin of the crown.  If this occurs and the situation is not rectified quickly it can lead to the fabrication of a new crown.

Unencumbered teeth like to migrate like birds.  They like to move towards the front of your mouth.  In addition they like to move up or down depending on if they are on your upper or lower jaws.  This ability for teeth to move is why orthodontics or braces work.

When you crown is no longer attached to your tooth there is now a space both horizontally and vertically for the tooth to begin moving into.  So the quicker the crown is restored the less chance of movement of the opposing and/or adjacent teeth.

Do you want to be Crowned?

Prince Charles did and did.  He now has a crown and is King.  In dentistry we can also crown you.  However, you would rather avoid being crowned for it means your tooth is in trouble.  Sometimes a crown may be too aggressive and instead you need a veneer.  Does the King of England need a veneer, he might look a little better.

So to crown or to veneer that is the question.  Generally when you get a cavity or decay in a tooth due to lack of brushing or flossing the first step is to place a filling.  Think of this as a pothole in the road.  If the pothole is not fixed as cars continually drive over this hole becomes larger.

 In your mouth a hole if not repaired will also get bigger and if left long enough could become painful.

So initially a filling is placed.  If you get another cavity in the same tooth or if the filling breaks then it’s time for your tooth to be crowned.

Think of a crown as a girdle to hold your tooth together.  Today the crowns are made of esthetic materials that will match you adjacent teeth.  Your tooth is now whole and you’re ready for action.  Sometimes on anterior teeth a hole is bigger than a filling but not big enough for a crown.  In this case we can place a veneer to replace the missing or decayed portion of your tooth.

To Inlay to Onlay to Crown

Dentistry is the filling of holes, holes you create within your teeth. When you eat food and do not clean it off the food breaks down and releases acids which attack the outside surface of your teeth.  Your teeth are covered with a very hard surface called enamel.  It is so hard that we have to use industrial diamonds to cut it.  The enamel around your teeth represents the hardest substance in your body.

 But bacteria in your mouth produced by the breakdown of food are tenacious and will attack your enamel until they break through.  You now have a cavity or hole within your tooth and once inside you’re no longer able to clean the bacteria away on your own.  

 The inside of your tooth is made up of a substance called dentin which is much more porous and susceptible to decay.  Anaerobic bacteria which form tooth decay like environments which are warm, dark and without air.   This is the exact situation within your mouth.

 As the bacteria multiply they get hungry and begin to eat your tooth.  If the hole produced is small we can place a filling.  Filings are made of gold, silver and now mostly composite which matches the color of your tooth.  If the decay becomes too large we can have a lab processed filling of gold or porcelain to replace the missing tooth structure.

 For a filling to be placed we need to have more tooth than filling to work with.  In a lab processed filling we still have more tooth than filling but the filling has to be fabricated in a lab to restore the additional contours of your tooth which you have lost.

 If the filling becomes too large a crown has to be made which will restore the tooth to its original condition by acting as a girdle to hold the tooth together.  The crown can be gold or tooth colored porcelain.

 The sooner a cavity is detected the smaller the restoration necessary to resolve the situation.

To be or not to be is not the question rather what size restoration will need to be done for the health or your tooth.

Can I Ignore a Cracked Tooth?

If you see a crack you must react. There is a difference between a craze line and a crack.  Everyone has craze lines on their teeth.  If you hold a light up to your teeth you will see fine lines on the surface of the teeth.  This is caused over time due to temperature differentials when you eat.   You bite into an ice cream cone and the ice cream is below 32 degrees.  You then immediately drink some coffee which could be at 90 degrees.  The immediate difference in temperature causes these lines to occur.

A crack however is much deeper and more apparent.  If you bite down on something and your tooth starts to hurt you may have a crack.  If you’re a grinder or clench meaning you bite down hard on your teeth or move your jaw back and forth while holding your teeth together a crack can only become worse.  Clenching and grinding is due to stress and if you can’t remove the stress we can help with a night guard.   

It’s like when your car’s windshield is hit with a rock sometimes it causes a chip while at other times the chip expands into a crack which progresses.  This is the condition with your teeth.   Sometimes a chip turns into a crack and can spread.  In a minor case it is seen and repaired quickly.  Left alone it can expand into the nerve requiring root canal therapy.  In the worst case it can fracture your tooth in half requiring the removal of the entire tooth.

Help I’ve Lost My Kingdom...I lost my crown!

My crown has come off.  What will I do?   I am still the master of my universe but inside my mouth I don’t have a clue.  Yes some dentists say crowns are permanent but even your dental insurance carrier will pay for the same procedure every five years, so much for permanence.  Your crown should last for more than five years but crowns do come off.

 If you crown comes off the first thing to do is NOT to place it back in your mouth.  You need to call a dentist – maybe me and bring the crown and yourself to the office.  Hopefully when your crown came off it did not break the tooth which is supporting it.  

 In most cases we can clean the crown and the tooth and then reattach it back to how it was.  The reason you do not want to place it yourself in your mouth with temporary cement from the drug store is if you do not place it exactly correctly you may not be able to close your mouth.  Also sometimes the crown comes off due to decay and needs to be remade.

 The most important consequence of placing it back into your mouth on your own is that you might swallow it.  If it goes into your stomach it will probably pass through but you will have to look then next time you see a man about a horse.  If it drops into your windpipe you’ll be at the emergency room.

Dental Implants

Implant as in placing.  You implant a seed into the ground and eventually a plant arrives.  We plant an implant into your jaw and eventually a new tooth arrives.  What is a dental implant?  Well, it is a piece of titanium which most people can tolerate that is placed into your jaw to simulate the root of a missing tooth. 

Life happens and sometimes a tooth needs to be removed.  Most people do not want the picket fence look in their mouth.  When a tooth is missing since there is no light inside your mouth from the outside you see a black hole.  For a very long time in dentistry the main way a missing tooth was replaced was by doing substantial work on the adjacent teeth so that the missing tooth could be replaced by attaching it to the adjacent teeth. 

The adjacent teeth generally do not want to participate in this procedure.  They are happy just being left alone.  But sometimes the needs of the one outweigh the needs of the many.  I sense Mr. Spock here.  Today we can place a single crown without disturbing the adjacent teeth.

So, we place a piece of titanium into your jaw after removing the broken or infected tooth and we let it sit there for some time.   After a period of four months the implant has taken up residence in your jaw and is ready to be restored.  At this point we can place a crown which resembles the missing tooth into your mouth and you’re good to go.

Fun Floss Facts Worth Knowing

Floss is strong. You’d be hard pressed to break it with just your hands.  Waxed floss is easier to use since it remains in a single strand.  Unwaxed can fray apart.  Flossing can be frustrating since you need to coordinate your fingers with the floss.  If you have a small mouth and big fingers, flossing is more challenging.  Flossing is good for your teeth and good for your gums.  Hard to get a tooth brush between your teeth – it just won’t fit.

If you have braces or a fixed wire in your mouth after orthodontic treatment flossing will be problematic.  For those individuals two to three in office cleanings per year are required to keep your gums healthy.

Trends in Dentistry

Over the decades dentistry has changed.  When I first started practicing there was more dental decay present in every patient.  The main culprit was the lack of fluoride in the public water system.  It was routine to see patients with cavities in almost every posterior tooth.  At that time the choices were silver or gold.   For most patients gold was too prohibitive in cost so silver was the only option.

Many other patients had severe periodontal disease necessitating the removal of their teeth and the placement of full or partial denture or plastic teeth to restore the missing spaces.  Alternatively fixed bridges or connected crowns or caps could be made to replace missing teeth.

Today things have changed.  Due to the presence of fluoride in the drinking water and better hygiene by the general public the level of decay has decreased.  Also advancements in dentistry have led to the creation of more esthetic alternatives to fix your teeth.  White fillings have replaced silver or metal fillings and implants can restore a missing tooth without damaging the adjacent teeth.

Periodontal procedures can be applied to strengthen the gums holding your teeth into your jaws.  Whitening and cosmetic procedures can enhance your appearance.  The saying you lead with your smile is readily apparent.  If you are not happy with your smile you will probably not smile.  Smiling helps to improve your outlook and positivity.

How Long Can A Dental Crown Last?

Dental crowns or sometimes called caps by the general public are used to restore teeth with large decay or larger broken fillings.  The crown helps to strengthen the damaged tooth and allow it to return to normal function.  Crowns are also used after a tooth has a root canal procedure wherein the nerve of the tooth has been removed due to infection leaving the tooth vulnerable to fracture.  The crown helps to keep the pieces of the tooth together.  If an individual loses a tooth and undergoes an implant which is a way of totally replacing a missing tooth the crown restores the part of the tooth you see in your mouth. Crowns can also be used for cosmetic reasons such as to replace the color of yellowed or stained teeth or teeth which are too small or misshapen.

Dental crowns are made of different materials.  In the past crowns would be made of gold.  Gold is very biocompatible with the mouth and wears the most similar to your actual teeth.  This prevents the tooth opposing the crown from wearing evenly over time.  Today most individuals will not accept the yellow color of gold so most crowns now contain porcelain which can be matched to the color of the patient’s teeth. Porcelain fused to metal gives the esthetics of porcelain with a metal substructure to support the tooth.  This is especially useful in teeth which have undergone root canal therapy and need additional support.  Today there are also crowns that have porcelain on the outside but instead of metal core a ceramic one is used to support the porcelain.  The advantage of this combination is that the crown can be even more esthetic and is a good choice for patients who have metal allergies.

Crown treatments require two appointments.  The first is when the actual tooth is put back together and an impression is taken to send to the lab which makes your final crown restoration.  On the second appointment your new crown is bonded onto the prepared tooth structure remaining in your mouth.  During the period between the two appointments which is generally several weeks a temporary restoration is placed in your mouth that allows you to continue to function normally.

So how long does a dental crown actually last?  Most crowns will last for many years.  A gold crown could last a lifetime.  The problem with crowns versus your own tooth is that your own tooth is a complete structure while the crown is like a hat on your head.   There is a crown margin all around your tooth where the crown ends and your tooth begins.  This area over time can develop decay causing the crown to fail.  Your part in this scenario is to brush and floss your teeth twice a day every day.

You need to keep this crown margin clean.  Also since porcelain is glass and you will apply a tremendous pressure to it when chewing there is the possibility of the porcelain breaking.  A crown prep is a large restoration on your tooth and sometimes the nerve within the tooth will die or become infected.  If this happens a root canal will be needed which can reduce the longevity of your tooth.

Insurance companies which do not want to pay for re-treatments believe if a crown lasts five years it is a success.  However crowns can last anywhere from 15 to 20 years.   At this point the crown may still be successful but from an esthetic standpoint the patient may want to replace it.

The cool thing about gold is when you melt it. It looks like mercury except it is gold in color. You can actually see your reflection in it and before it becomes liquid it has a reddish golden sheen. Just magnificent and it makes you realize why it has been in use since antiquity. However, at 1948 degrees Fahrenheit you certainly don’t want to touch it for then you’ll definitely be singing, but not in a good way.
